It actually is a fanon thing that everyone (into them) just went on board with. There's this joke in the fandom that Sylus and Raf are the entry level to the monsterfucker pipeline, and it starts by getting used to the fact that dragons and mermen can have two dicks. Of course, things could be way more interesting anatomically, but lads fandom isn't known for being particularly daring or exhaustive in the kinky writing department (we'll see with this year's kinktober, who knows).
